Continuing Education Credit Forms
Bench to Bedside Series
The American Association of Colleges of Nursing will award
one and a half (1.5) contact hours of continuing education credit for each of
the ten videotapes/CD-ROMs in the Bench to Bedside series. From 1.5 to 15
contact hours may be earned. A Continuing Education Credit Form for each
program in the series is located at the links listed below. Click on the title
of the program for each form you would like to view and print.
The viewing of each tape/CD is considered a separate educational activity, so a separate Continuing Education Credit (CEC) Form must be submitted for each tape/CD viewed. Continuing Education Credit Forms may be submitted one at a time or several at the same time. When a tape is viewed by a group, each individual must complete a form but the forms can be mailed together. You may photocopy the form for distribution to an entire group of viewers, but please make sure that each viewer makes his/her own copy of the completed form before mailing or faxing it.
A Continuing Education Credit Form must be submitted to verify you viewed the tape and therefore earned the credit.
Steps
1. Find the program title you want from the list of links below and click on it.
2. Print a copy of the Continuing Education Credit Form for that program using your browser's "print function."
3. View the videotape or CD-ROM of that program
4. Complete all information requested
5. Copy the completed form for your records of contact hours earned. No additional verification will be sent to you.
6. Send original of the completed form to:
